Practical examples of impact

Case note / 01

Acute mental health discharge protocol

Designed a protocol with acute mental health units that stopped people being discharged straight into homelessness. Created a clear pathway into respite and move-on accommodation, helping people become tenancy-ready instead of returning to crisis.

Case note / 02

Custody release accommodation planning

Worked closely with prisons and probation to ensure people leaving custody had planned accommodation routes and could secure their legal rights under homelessness applications — reducing crisis presentations and improving safety.

Case note / 03

MAPPA case: property restrictions & income barrier

Supported a MAPPA client who couldn’t return home due to licence conditions and had no income to access private rented accommodation.

Resolved this by:

securing an agent to rent out his property
demonstrating to senior management that this created a viable income
arranging for the council to use the property to house a homeless family
negotiating incentives with a reluctant landlord

Outcome: two homeless households housed, significant interim accommodation costs avoided, and the client gained a stable income and safe accommodation.
